Manisha Koirala was born in Kathmandu, Nepal, to the politically powerful Brahmin Koirala family. Prakash Koirala, her father, is a politician and a former environment minister, while Sushma Koirala, her mother, is a housewife. Siddharth Koirala, her actor brother, is her only sibling.
Several members of her family went on to become politicians, including her grandpa, Bishweshwar Prasad Koirala, and two of her great-uncles, Girija Prasad Koirala and Matrika Prasad Koirala, who served as Prime Minister of Nepal from the late 1950s to the early 1960s.
