Perkins was the son of stage and screen actor Osgood Perkins and his wife, Janet Esselstyn, and was born in Manhattan, New York City. Andrew Varick Stout Anthony, a wood engraver, was his paternal great-grandfather. When his father died, he was five years old.
Perkins was a descendant of John Howland, a Mayflower passenger. After moving to Boston in 1942, he attended Brooks School and Browne & Nichols School, as well as Columbia University and Rollins College.

Perkins stayed married to Berenson until his death. Perkins’ widow perished in the September 11 attacks onboard American Airlines Flight 11 in 2001, a day before the ninth anniversary of his death. Following a vacation on Cape Cod, she was returning to her California home.

Perkins was diagnosed with HIV while filming Psycho IV: The Beginning, and died on September 12, 1992, at the age of 60, from AIDS-related pneumonia at his Los Angeles home.
