Upendra was born in Bangalore to Manjunath Rao and Anusuya. His father was from a Kannada family from Thekkatte, Koteshwara, Kundapur Taluk, Udupi district, and Karnataka (previously Mysore State).

Sudheendra Rao is his older brother. Rao came from an impoverished household. He had an eye condition as a child, which can be observed in numerous cinematic situations when he rolls his eyeballs.
